Confined space fatalities follow a pattern with grim consistency: the first person is overcome, and the people who die alongside them are the ones who went in to help. That pattern is the reason the standby person must not enter and the reason a rescue plan has to be non-entry by default. A permit that captures gas readings but leaves rescue as "call 111" has not addressed the mechanism by which people actually die in these spaces.

Stratification is real — a space can be safe at the opening and lethal at the base. The permit requires testing at multiple depths and continuous monitoring during occupancy.
A valve that is closed is not isolated. The permit requires the isolation method, its verification, and the lock or tag reference.
The permit controls the entry. It does not replace training, gas detector calibration, or the rescue capability itself.

Confined space entry in New Zealand is governed by the general risk-management duties together with AS/NZS 2865, which sets the recognised practice for identification, entry control, atmospheric testing and rescue.
